<p>BIA-ALCL or breast implant-associated, a typical large cell lymphoma is a tumour condition that develops in the capsule surrounding breast implants. It's incredibly rare and it's most akin to non-Hodgkin's lymphoma.</p>
<p>It is actually a cancer, but it develops from what's thought to be an interaction between the textured surface of some breast implants and the body's immune response to that breast implant. So it definitely does not happen with every breast impact, the more textured the breast implant, the more likely you are to develop this very rare cancer.</p>
<p>However, even with textured implants, it's not a given. So the rights vary according to how textured those implants are and can be as common as one in 3,000 or thereabouts, out to one in 87,000 or thereabouts. So with a range that's that divergent, there's obviously a lot of room for interpretation and confusion. Particularly in the eye and mind of the public.</p>

<p><em><strong>Plastic surgeons</strong>&nbsp;<strong>spend 8-10 years of training after leaving medical school to become surgeons</strong></em>. Plastic surgeons are trained by the Australasian College of Surgeons, which was formed in 1927. This training is recognised the Australian Medical Council, which is authorised by the Australian Federal Government to certify surgical training.&nbsp;<em><strong>At the completion of plastic surgery training the college awards the&nbsp;</strong></em>surgeon<em><strong>&nbsp;the title of Fellow of the Royal Australasian College of Surgeons (FRACS)</strong>.&nbsp;<strong>To join the Australian Society of Plastic Surgeons (ASPS) one must have a FRACS qualification</strong>.</em>During the intensive years of surgical training, plastic surgeons are taught how to treat patients with a wide range of reconstructive and cosmetic surgical operations.</p>
<p><em><strong>In contradistinction, any doctor can call themselves a cosmetic surgeon</strong></em>. The basic university medical degree confers the graduate with a Bachelor of Medicine Bachelor of Surgery (MBBS). Historically cosmetic surgeons have referenced their basic medical degree qualification as a licence to operate.&nbsp;<em><strong>Cosmetic surgeons have a far narrower skill set than plastic surgeons.</strong></em>&nbsp;Until 1999 doctors interested in cosmetic surgery would arrange an apprenticeship-type situation, so as to watch/assist in procedures, thereafter calling themselves a cosmetic surgeon. In 1999 the Australian College of Cosmetic Surgery (ACCS) was formed to try to bring greater consistency to the experience gained in this apprenticeship-type learning model but&nbsp;<em><strong>the ACCS is not&nbsp;</strong></em>recognised<em><strong>&nbsp;a specialist training body.</strong></em></p>

We thought we&#8217;d zoom in now on breast augmentation surgery. Breast augmentation has become increasingly popular in recent years with more and more operators performing the procedure, and invariably this has led to more and more complications. Whether they&#8217;re called botched procedures or surgical mishaps, there are many unfortunate patients out there in the community who&#8217;ve had breast augmentation surgery and rather than ending up with a dream result, have ended up with sorrow, misery, sometimes pain, and dissatisfaction.

To set these misadventures right takes much experience and much planning, and they can be corrected, but it&#8217;s not a simple thing. We need to take into account the patient&#8217;s tissue features, whether they&#8217;re more likely or less likely to have a recurrence at the time of revision surgery. We need to factor in the size of the implant, and was it appropriately chosen for the patient, where was the implant placed? Is it above or below the muscle? What is the muscle doing? That&#8217;s the pectoralis major muscle on the chest wall. Has it retracted? Does it need to be resuspended or dropped down? Does the implant need to be placed in a different pocket? Does a different shape implant need to be chosen? Or, do we need to start from scratch because the pocket is too wide or too low? Has there been stretching of the lower pole of the breast? Has there been distortion of the nipple areola complex? And so on and so forth.

Certainly, in terms of altered sensation to the nipple areola complex, this is usually a temporary phenomenon, but it can be a permanent one in a very small percentage of patients, and this is probably the most difficult thing to attend to and really does not have a satisfactory surgical solution. To that end, we&#8217;ll continue to delve down into our revision breast surgery procedures in this series of videos, but for now I&#8217;ll sign off. Today we&#8217;re talking about revision breast surgery. They&#8217;re many, many women having cosmetic breast operations in this day and age, which is a wonderful thing for self esteem and to achieve one&#8217;s ideal body image.

However, they do come at a potential costs. Now, problems happen with every operator and every operation. But they certainly occur more commonly when performed by inexperienced hands, and more problems are seen when these complicated surgeries are performed in the day surgery setting rather than a formal hospital environment.

Needless to say, they do occur. When they do, they need to be made right. This can be very difficult, but so long as you fall back on the principles found in sound, plastic reconstructive surgery, then cosmetic mishaps can be made right again.

Revision surgery comes in many shapes and forms, essentially any procedure that can be performed in the cosmetic or even reconstructive realm has the potential for problems. And those problems often lead to reconstructive surgery. So revision surgery down the track from these prior procedures is often more complicated than the original one was and takes a lot of forethought, planning, and experiences to right.</p>
